Details of Experience:
Susan is a woman who enjoys spending time with her children and grandchildren. Surrounded by the love of her family, Susan was eager to return home. Ms. French spent some time in St. Charles hospital, following a diagnosis of Hypoxemia. Hypoxemia happens when there is a low level of oxygen in the blood causing things such as headaches and shortness of breath. After her time in the hospital, Ms. French was experiencing some weakness and trouble performing her normal activities of daily life. She was recommended to go to a Subacute Rehabilitation to gain strength and her independence back before returning home.

After careful consideration, Susan chose Water’s Edge Rehabilitation and Nursing Center at Port Jefferson with the help of her family. Susan arrived at Water’s Edge on December 28th for all her rehabilitation needs. When she arrived, she was greeted by our admissions nurse who escorted her upstairs to her room. After entering her room, she was evaluated and introduced to her CNA, who would be taking care of her for the evening. The next morning, she was greeted by several members of our interdisciplinary team at Water’s Edge.

Our therapy department evaluated Susan on day one. Susan’s goal was to “get stronger and return home.” According to her therapists, she demonstrated good rehab potential, because she can make her needs known, she has a supportive family, and the motivation to return home to her prior level of function. Susan was ready to put in the work to get where she needed to be.

Susan worked daily with her physical therapist’s Danielle and Jamie. Together they worked on seated therex, standing therex, transfer training, stair, and balance activities. They concentrated on strength training, and functional mobility training so that she could return home safely. She began her stay at Water’s Edge needing moderate assistance with her transfers and only being able to ambulate 15 feet with assistance. After only a week of working with our therapy team, she was progressing tremendously. She also worked with her Occupation therapist, Nichole. Together they did resistance exercises and therapeutic exercises to facilitate independence in self-care tasks. They worked on transfer training and balance skills, as well as throwing/catching activities to facilitate strength, coordination, and balance. She began her stay needing assistance with her activities of daily life. After only a week of working with Nichole, Susan noticed a difference in her abilities!

Ms. French spent 17 days at Water’s Edge, working hard every day to meet her goals. Upon discharge, she was performing her activities of daily life independently, as well as transferring and ambulating independently. Susan and her family were thrilled with her progress and ecstatic that she was able to return home so quickly.
